Barely touching 90 on the gun...but mostly living at 88-89. Not good

That's slightly faster than his previous game though. Still, it surely seems like there is plenty of reason to remain concerned.

But you're missing one thing, Randy; the movement on his pitches!
That wasn't there in his first start.
IMHO, you're putting way too much emphasis on what the radar gun says.
Especially in Cueto's case.

But the point is before the elbow troubles he was roughly 5 mph higher on average...THAT is a concern

Your radar and Krukow's seem to be different.
Everything I've read, everything I've heard, not just from Krukow but others, said Johnny was topping out at 91-92. Not that different than his last start.

Topping, yes...and before his injury he was topping at 96. That was his "reach back" velocity, as Krukow puts it.
As for the majority of his fastballs, Johnny was normally at 92-93 before the injury, now he is at 88-89 most of the time.

Not arguing with what you saw. I saw the 90-91, too.
But my comment was about what Johnny was prior to the injury.
92-93.
1-2 mph difference, if you can PITCH, and he can, isn't that big of a deal.

Here are the average velocities for Johnny beginning with 2010:
2010 -- 93.2
2011 -- 93.4
2012 -- 92.7
2013 -- 92.5
2014 -- 93.1
2015 -- 92.5
2016 -- 91.5
2017 -- 91.3
2018 -- 89.7
It may differ from pitcher to pitcher, but I believe there is a velocity below which it becomes really hard to pitch well. The good news is that pitchers DO sometimes adjust.
